1. Agreement to Terms

By accessing or using amplifyhealth.com (the "Site") or any services provided by Amplify Health ("Amplify," "we," "us," or "our"), you agree to be bound by these Terms and Conditions ("Terms"). If you do not agree, do not use the Site or our services.

2. Eligibility

You must be at least 18 years of age to use our services. By using the Site, you represent and warrant that:

• You are at least 18 years old
• You are a resident of the United States in a state where Amplify Health operates
• You have the legal capacity to enter into a binding agreement
• You will provide accurate, current, and complete information during registration and intake

3. Nature of Services

Amplify Health is a telehealth platform that facilitates connections between patients and independent licensed physicians and pharmacy partners. Specifically:

• We provide a platform for physician-supervised programs including hormone optimization (TRT and menopause support), weight management, sexual enhancement, and longevity
• Each practice branded as Amplify works with a network of independent licensed physicians. Amplify Health does not employ or own a medical practice
• Compounded medications are dispensed by licensed compounding pharmacies within our network
• All prescriptions are issued at the sole clinical discretion of the treating physician

Amplify Health does not practice medicine. We do not diagnose conditions, prescribe medications, or provide medical advice directly. Those functions are performed exclusively by independent licensed physicians.

4. Telehealth Services

By enrolling in a program, you acknowledge and agree that:

• Telehealth services involve delivery of healthcare via electronic communications and may differ from in-person care
• Certain treatments may require a live video consultation with a licensed physician before a prescription is issued; others may not require a live consult based on qualifying clinical factors
• Clinical decisions, including whether to prescribe, are made at the physician's sole discretion
• You should maintain a relationship with a primary care provider for general health needs
• In a medical emergency, call 911 or go to the nearest emergency room immediately

5. Compounded Medications

You acknowledge and agree that:

• The medications available through Amplify Health are compounded preparations dispensed by licensed compounding pharmacies, not FDA-approved drugs
• Compounded medications are legal under federal and state law when prescribed by a licensed clinician and prepared by a licensed compounding pharmacy
• Compounded medications have not undergone FDA review for safety, efficacy, or quality as finished drug products
• Individual results vary; no specific outcome is guaranteed
• You must disclose all current medications, supplements, and health conditions to your physician to avoid potential interactions

6. Account Registration

To access our services, you must create an account. You agree to:

• Provide accurate, complete, and current information
• Maintain the security of your account credentials
• Notify us immediately of any unauthorized access to your account
• Accept responsibility for all activity under your account

• Medications are shipped in discreet packaging

9. Prohibited Conduct

You agree not to:

• Use the Site for any unlawful purpose
• Submit false or misleading information during intake or consultation
• Share your account credentials or prescription medications with others
• Attempt to resell or redistribute compounded medications
• Circumvent or interfere with security features of the Site
• Use automated tools to scrape or extract data from the Site
• Impersonate any person or entity

10. Intellectual Property

All content on this Site, including text, graphics, logos, images, and software, is the property of Amplify Health or its licensors and is protected by applicable intellectual property laws. Unauthorized reproduction or distribution is prohibited.

11. Third-Party Services and Links

Our platform uses third-party services including gen-health.app (telehealth infrastructure), licensed pharmacies (pharmacy fulfillment), Stripe (payment processing), partner diagnostic laboratories, analytics providers, and LegitScript (compliance certification). We are not responsible for the practices of third-party providers. Your use of third-party services may be subject to their separate terms.

12. Disclaimer of Warranties

THE SITE AND SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT.

Individual experiences featured on our site are illustrative and are not a guarantee of results.

13. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, AMPLIFY HEALTH SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE SITE OR SERVICES.

OUR TOTAL LIABILITY TO YOU SHALL NOT EXCEED THE AMOUNT YOU PAID TO US IN THE THREE (3) MONTHS PRECEDING THE CLAIM.

We reserve the right to modify these Terms at any time. We will notify you of material changes by posting the updated Terms on this page. Continued use of the Site after changes constitutes acceptance.
